ADMISSIONS CRITERIA

Program Eligibility Requirements

Applicants must meet the following eligibility requirements before being considered for admission to the I&R CARES Shared Housing Program.

Basic Eligibility

Applicants must be 18 years of age or older, be legally authorized to reside in the United States or otherwise eligible to enter into a housing agreement, and meet the program's basic admission requirements.

Independent Living Ability

Applicants must be able to perform daily living activities independently, manage their own medications, maintain personal hygiene, keep their living space reasonably clean, and safely evacuate the residence during an emergency

Income & Community Living

Applicants must have a consistent and verifiable source of income, be willing to pay rent and program fees, and demonstrate the ability to live respectfully with others while following house rules and community standards.

Health & Safety Requirements

Applicants must be physically and mentally able to live safely with minimal support, must not require 24-hour supervision or skilled nursing care, and must provide the required documentation to complete the admissions process.

STEP-BY-STEP PATHWAY

The Intake & Move-In Process

Complete each step of the admissions process before placement into the I&R CARES Shared Housing Program.

Referral & Application

Submit a referral through a hospital, social worker, case manager, veterans organization, community agency, family member, or complete the Resident Application to begin the admissions process.

Eligibility Review

Complete the Client Eligibility Questionnaire. Our team reviews your eligibility based on age, independent living ability, income, health and safety requirements, behavioral expectations, and program availability.

Document Verification

Provide the required documentation, including:

  • Government-issued photo ID
  • Proof of age
  • Proof of income or benefits
  • Medical documentation (diagnosis, treatment plan, medication list)
  • Emergency contact information
  • Any additional documents needed to verify eligibility

Orientation & Move-In

Approved applicants complete resident orientation within the first 24 hours of move-in. Orientation includes reviewing the Resident Handbook, House Rules, Emergency Procedures, Curfew Policy, Visitor Policy, Safety Protocols, and signing all required acknowledgment and consent forms.

Required Documents for Intake Processing

Government-Issued Photo Identification

Provide a valid government-issued photo ID to verify your identity. Examples: Driver's License, State ID, Passport, or Military ID.

Proof of Age

Documentation confirming that you are at least 18 years of age.

Proof of Income or Benefits

Provide documentation showing a consistent and verifiable source of income. Examples: Employment income, SSI, SSDI, retirement benefits, veterans benefits, or other documented income.

Emergency Contact Information

Provide the name and contact information of someone who can be reached in case of an emergency.

Additional Supporting Documents

Additional documentation may be requested if reasonably necessary to verify eligibility or complete the admissions process. .

Completed Intake Forms

Applicants will be asked to complete the required intake forms, consent agreements, emergency contact forms, and policy acknowledgment forms during the admissions process.

House Rules & Program Expectations

Residents are expected to follow program policies and community standards to help maintain a safe, respectful, and supportive living environment.

House Rules Summary

Key policies for safe community living

  • Curfew Policy: Sunday–Thursday: 10:00 PM | Friday–Saturday: 11:00 PM. Curfew extensions may be requested for approved reasons with advance approval whenever possible.
  • Quiet Hours: Sunday–Thursday: 9:00 PM – 7:00 AM | Friday–Saturday: 10:00 PM – 8:00 AM. Residents are expected to minimize noise and respect others.
  • Visitor Policy: Visitors are welcome daily from 10:00 AM – 8:00 PM. All visitors must sign in, remain in approved areas, and follow house rules.
  • Drug & Alcohol Policy: Illegal drugs, non-prescribed controlled substances, alcohol (where prohibited), and drug paraphernalia are not permitted.

Program Expectations

Resident responsibilities throughout the program

  • Weekly House Meetings: Attend scheduled house meetings unless excused. Meetings cover safety, house updates, resident concerns, community resources, and upcoming events.
  • Respectful Community Living: Treat residents, staff, neighbors, and shared property with respect while following all house rules and community standards.
  • Resident Orientation: Complete orientation within the first 24 hours, including review of the Resident Handbook, emergency procedures, safety protocols, and required forms.
  • Resident Acknowledgment: Sign all required policy acknowledgments, intake documents, emergency contact forms, and consent agreements before or during move-in.

Who is eligible to apply for the Shared Housing Program?
